DOCUMENTS REQUIRED FOR AN ATHLETE LICENSE (For those obtaining a license for the first time):
- Photocopy of ID,
- Health Report,
- Parental Permission Form (for those under 18), It is not necessary to physically print the Parental Permission Form generated via e-Government.
- Two photographs,
- Document proving club affiliation for a club visa (Not required for individual licenses).

- Photocopy of ID,
- Health Report,
- Parental Permission Form (for those under 18), It is not necessary to physically print the Parental Permission Form generated via e-Government.
- Document proving club affiliation for a club visa (not required for individual licenses)
- Receipt proving visa fee payment.
Prepared documents are submitted to the Provincial or District Directorates of Youth and Sports, and the license or visa process is completed.
Athlete visa applicants:
- Children of martyrs
- Children staying in Love and Children's Houses affiliated with the Ministry of Family, Labor and Social Services of the Republic of Turkey
- All disabled individuals with a minimum 40% disability report
- Athletes over the age of 65 are exempt from the visa fee.
Students under the age of 18 (under 19) who obtain a license from their school's sports club will not be charged a visa fee, provided they present a student document proving their affiliation with the school to which their school's sports club is affiliated. Licenses and visas are free of charge.
Athletes who will obtain a club license must obtain a certified document proving their affiliation with the club from their club.
Athletes who will transfer from a club to an individual license or change clubs: They must obtain the consent form from their club.
Creating a Referral for an E-Government Health Report
Go to e-Government → Type "sports information system" in the search bar. → Click "Sports information system" → Go to the application → Click "Health Transactions" → Click "Referral for a Health Report" → If you will be creating a referral for a person under your guardianship, click "Transfer to my guardianship" → Enter your Custody ID and Date of Birth and click "Complete the Transfer" → Click "Create a New Referral" → Select the Province. Click "Add Health Referral" → After creating the referral record, you can obtain the health report from any healthcare institution in any province, regardless of the province you select. The referral request you create is valid for 6 months.
